By signing below, I affirm that the above information is true and accurate, I give permission for my child to participate fully in Nxt Level Fitness training, and I release Nxt Level Fitness/Rob Brown of any liability should my athlete or any person on site with my athlete (e.g., parent, sibling, etc.), sustain any injury or other negative consequence while training with or otherwise on site at Nxt Level Fitness.
