PLEASE READ CAREFULLY THE CONTENTS OF THIS ELECTRONIC WAIVER BEFORE SIGNING.
The undersigned participants hereby waive all claims against any and all persons associated with the Palmetto Charter School Mommy Son Event. I understand that I am participating in an organized kickball game and other games and activities, which require physical activity and have potential body contact. I also understand that the below-listed student(s) assume the same risks and I hereby assume full responsibility for their actions and waive any liability from PCS.
I assume full responsibility for all my actions during and connected to the above organization (Palmetto Charter School) and event (Mommy Son Night). I understand the risk of participating in this form of physical activity and hereby release Palmetto Charter School and all of its agents (including the PTO and any volunteers), employees and associates of and from any and all liability, claims, demands, actions, medical bills and causes of action whatsoever arising out of or relating to any negligent or other act or omission, and/or any loss, damage or injury, including death, that may be sustained by the undersigned or any property of the undersigned in participating in this form of activity. I, the undersigned also state that I am in good physical condition and know of no reason why I cannot or should not participate. I understand that in case of emergency, I hereby authorize any licensed medical personnel to perform any accepted medical assistance deemed necessary and I agree to bear the expense of any such treatment.
I am aware that the location where Palmetto Charter School's Mommy Son Night is being held, Blackstone Park, has a playground onsite. The playground is not part of Mommy Son Night and therefore all participants should stay off the playground. I agree to waive any and all liability if my child(ren) or I choose to play on the playground and succumb to any injury.
In signing the foregoing release, the undersigned hereby acknowledges and represents: that he/she has read the foregoing release, understands it and signs it voluntarily; that he/she is over 18 years of age and of sound mind. The undersigned parent or adult over the age of 18 is signing for the below PCS students that will be participating in the event.
