    BEFORE SIGNING.

  • I. Parties and Activities Covered

  • This Waiver covers the Participant's involvement in all activities, programs, events, use of facilities, and services offered by, or affiliated
    with, Bridges Center, including but not limited to:
    1. Recreational/Physical Activities: Sports (basketball, volleyball, etc.), games, exercise, field trips, outings, and transportation to
    and from events.
    2. Fellowship/Spiritual Activities: Worship services, Bible studies, prayer meetings, small group discussions, and community
    service projects.
    3. General Center Use: Use of facilities, equipment, computers, kitchen, and common areas.

  • II. Assumption of Risk

  • I, the undersigned Parent/Legal Guardian, understand and acknowledge that participation in Center activities involves inherent and
    foreseeable risks, including risks that may result in bodily injury, illness, property damage, or death. These risks include, but are not
    limited to:
    • Physical exertion, falls, and collisions during sports or games.
    • Injuries from the use of equipment, furniture, or facilities.
    • Risks associated with travel and transportation.
    • Risks associated with group settings and the conduct of other participants.
    • Risks associated with outdoor activities, automotive, construction, or garage area
    • Risks associated with the drop off and pick up of your child
    I knowingly and voluntarily assume all such risks, whether known or unknown, foreseeable or unforeseeable, even if arising from the
    ordinary negligence of the Center or its staff, volunteers, or agents, to the fullest extent permitted by Texas law.

  • III. Waiver and Release of Liability

  • In consideration for my child being allowed to participate, I hereby expressly waive, release, and discharge Bridges Center, Bridges
    Community Ministries, and their respective directors, officers, employees, volunteers, agents, and representatives ("Released Parties")
    from any and all claims, demands, causes of action, or liabilities arising out of or related to my child's participation INCLUDING CLAIMS
    CAUSED BY THE ORDINARY NEGLIGENCE OF THE RELEASED PARTIES, to the fullest extent permitted by Texas law.
    This release does not apply to acts of gross negligence, willful misconduct, or intentional harm. Accordingly, I:
    1. WAIVE AND RELEASE Bridges Center, its directors, officers, employees, volunteers, agents, and representatives (collectively, the
    "Released Parties") from any and all claims, liabilities, demands, actions, or causes of action arising out of any loss, damage, or
    injury, including death, that may be sustained by the Participant or property, Including, but not limited to claims caused by the
    ordinary negligence of the released parties or otherwise    , while participating in any Bridges Center activity.
  • 2. INDEMNIFY AND HOLD HARMLESS the Released Parties from any loss, liability, damage, or costs, including reasonable attorneys' fees, that they may incur as a result of any claim or lawsuit brought by or on behalf of the Participant due to the Participant's participation, whether caused by the Participant's own negligence or the negligence of ordinary the Released Parties.
    I understand that this waiver extends to all claims arising now or in the future.

  • IV. Medical Authorization and Consent

  • I authorize the Center staff or volunteers to seek and obtain medical care, including emergency medical treatment, hospitalization, or surgical care, for the Participant if, in the Bridges Center's sole discretion, such treatment is deemed necessary, for known allergies and current medications and I cannot be reached to grant consent. I agree to be responsible for all costs and expenses associated with such medical treatment.

  • V. Media and Publicity Release

  • I grant the Center permission to use photographs, videos, or audio recordings of the Participant, taken during Center activities, for the purpose of promoting the Center's ministry and mission (including use on websites, social media, and printed materials) without further compensation or notification. (Check one): If neither option is checked, permission is considered granted.

  • VI. TEXAS GOVERNING LAW AND VENUE

  • This Agreement shall be governed by and construed in accordance with the laws of the State of Texas. Venue for any legal action shall lie exclusively in the county in which Bridges Center is located.

  • VII. SEVERABILITY

  • If any portion of this Agreement is found unenforceable, the remaining provisions shall continue in full force and effect.

  • VIII. Understanding and Agreement

  • I certify that I am the Parent/Legal Guardian of the Participant, am over the age of eighteen (18) years, and have the legal authority to sign this Waiver. I have read this document carefully, understand its terms, and agree that no oral representations, statements, or inducements apart from this written agreement have been made.
