- Informed Consent and Acknowledgement
  
  I give permission for my child to participate in all activities associated with DBTR Camp, operated by BUCK DT LLC (“DBTR”), during the selected camp session.
  
  I understand that participation in sports activities, including softball, involves inherent risks. These risks may include, but are not limited to, minor injuries such as bruises or sprains, as well as more serious injuries such as fractures, concussions, paralysis, or in rare cases, death. I voluntarily assume all risks associated with my child’s participation, including travel to and from camp activities.
  
  In consideration of my child being allowed to participate, I release and hold harmless DBTR, its owners, coaches, staff, agents, representatives, volunteers, sponsoring agencies, and property owners from any and all claims arising out of my child’s participation in camp activities.
  
  In the event of an injury or medical emergency, I authorize DBTR staff to seek appropriate medical treatment for my child if I cannot be reached.
  
  I acknowledge that I have read and understand this consent and agree to its terms.
- Medical Release and Authorization
  
  As the parent or legal guardian of {athletesName}, I authorize DBTR Camp, operated by BUCK DT LLC (“DBTR”), to obtain emergency medical care for my child if necessary during camp activities.
  
  In the event of a medical emergency where I cannot be reached, I authorize qualified medical professionals to evaluate, diagnose, and provide appropriate treatment for my child. This may include emergency care, hospital treatment, diagnostic testing (including X-rays), and medically necessary procedures as determined by licensed medical personnel.
  
  I understand that every reasonable effort will be made to contact me or the listed emergency contact prior to treatment whenever possible.
  
  I also authorize DBTR directors, coaches, and designated staff to provide basic first aid and to arrange transportation to a medical facility if needed.
  
  This authorization is valid for the duration of the registered camp session.
  
  I sign this release voluntarily for the purpose of permitting emergency medical treatment for my child when I am unavailable.
