LIABILITY WAIVER AND ASSUMPTION OF RISK AGREEMENT
This Liability Waiver and Assumption of Risk Agreement (“Agreement”) is made in favor of Sammie’s Event Rentals, a division of The Templar Collective, LLC (hereinafter referred to as “Company”).

I understand that participation in Nerf activities hosted by the Company involves physical activity and the use of foam dart blasters and related equipment. I acknowledge that, although safety precautions and protective equipment are provided, participation involves inherent and unpredictable risks associated with recreational and physical activities.

These risks include, but are not limited to:

• Eye or facial injury from foam darts or equipment
• Slips, trips, falls, or collisions with participants or objects
• Strains, sprains, or other physical injuries
• Injuries resulting from rough play or failure to follow safety instructions
• Injuries related to the condition of the event venue or surrounding environment

I understand that protective eyewear and safety instructions will be provided, and agree that my child (or I, if signing for myself) will follow all safety rules and staff instructions. I understand that failure to follow safety rules may result in removal from activities without refund.

Parent/guardian or event host retains responsibility for supervising participating minors unless otherwise agreed in writing.

I acknowledge that the Company makes reasonable efforts to provide safe equipment and structured gameplay but cannot eliminate all risks associated with recreational activities.

Assumption of Risk
I voluntarily allow my child to participate and acknowledge that I fully assume all inherent risks associated with the activities described above.

Release of Liability
To the fullest extent permitted by law, I release and hold harmless the Company, its owners, employees, contractors, volunteers, and representatives from any claims, demands, damages, or liability arising from participation in these activities, including claims arising from ordinary negligence.

Venue & Property Acknowledgment
I understand that events may occur at private residences, parks, churches, or other venues not controlled by the Company. I acknowledge that the Company is not responsible for hazards, conditions, or supervision issues related to the event location.

Medical Authorization
In the event of an injury or medical emergency, I authorize Company staff to obtain emergency medical care for my child if I cannot be reached. I agree to be financially responsible for any medical treatment provided.

Behavior Agreement
I understand that safe and respectful behavior is required. The Company reserves the right to remove participants who engage in unsafe, aggressive, or inappropriate behavior.

Equipment Responsibility
I agree that participants will use equipment only as instructed and will not intentionally damage, modify, or misuse equipment.

Acknowledgment of Understanding
I certify that I am the parent or legal guardian of the participant listed below, or that I am signing for myself if over 18. I have read and understand this agreement and sign voluntarily
