DIE PRETTY LLC

TATTOO CLIENT CONSENT, RELEASE & FULL LIABILITY WAIVER

This agreement is entered into between the undersigned client and Sakana & Steel and Die Pretty LLC, including all owners, employees, independent contractors, apprentices, booth renters, and affiliates.

1. PREMISES LIABILITY (ENTERING THE SHOP)

I understand that from the moment I enter the premises of Sakana & Steel and Die Pretty LLC, I voluntarily assume all risks associated with being on the property.

This includes but is not limited to:

 Slips, trips, or falls
Injury from equipment, tools, furniture, décor, displays, cords, or flooring
Injury caused by other clients or third parties
Reactions to cleaning products or chemicals
Parking lot incidents
Property damage or loss

 I agree that Sakana & Steel and Die Pretty LLC are NOT responsible for any injury, damage, loss, or claim that occurs while I am on the premises.

 

2. ACKNOWLEDGMENT OF TATTOO PROCEDURE

I understand that tattooing involves piercing the skin with needles and depositing pigment into the skin.

I acknowledge risks including but not limited to:

Pain and discomfort
Bleeding
Swelling
Bruising
Infection
Allergic reaction to ink or products
Scarring
Keloid formation
Skin discoloration
Ink migration or fading
Unsatisfactory healing results

 

I voluntarily assume all risks associated with receiving a tattoo.

3. HEALTH DISCLOSURE

I certify that:

I am at least 18 years old and have provided valid government-issued identification.
I am not under the influence of drugs or alcohol.
I have disclosed all medical conditions including but not limited to allergies, skin conditions, blood disorders, immune disorders, pregnancy, medications, or any condition that may affect the tattoo process.

Failure to disclose medical information releases Sakana & Steele and Die Pretty LLC from all liability.

4. AFTERCARE RESPONSIBILITY

I understand that proper aftercare is my responsibility.

I agree to follow all written and verbal aftercare instructions.

I understand that failure to properly care for my tattoo may result in infection, fading, scarring, or other complications, and I release Sakana & Steele and Die Pretty LLC from any responsibility related to improper healing.

 5. NO GUARANTEE OF RESULTS

I understand that:

 Tattoo results vary depending on skin type, placement, lifestyle, and healing.
The final healed result may differ from the original design.
Touch-ups are not guaranteed unless otherwise stated.

 No specific outcome is promised.

 6. RELEASE OF LIABILITY

I hereby fully release, waive, discharge, and agree not to sue Sakana & Steel and Die Pretty LLC, including all owners, employees, independent contractors, and affiliates, from any and all claims, demands, damages, medical expenses, lost wages, attorney fees, or causes of action arising from:

 My presence on the premises
The tattoo procedure
Any complications arising immediately or in the future
Infection, allergic reaction, or skin reaction
Dissatisfaction with the final result
Property damage or loss

This release applies to any injury, illness, death, or damages whether caused by negligence or otherwise, to the fullest extent permitted by law.

 

7. INDEMNIFICATION

I agree to indemnify and hold harmless Sakana & Steel and Die Pretty LLC from any claims brought by me or on my behalf arising from the tattoo procedure or my presence in the shop.

8. PHOTO & VIDEO RELEASE

I grant permission for Sakana & Steel and Die Pretty LLC to photograph or record my tattoo and likeness for marketing, social media, website, and promotional purposes without compensation.

9. LEGAL ACKNOWLEDGMENT

I acknowledge that:

I have read this entire document.
I understand that this is a legally binding waiver.
I am signing voluntarily.
I understand that by signing this, I am giving up certain legal rights.
