Privacy and Disclosure
This notice explains how your health information may be used and shared and how you can access your records. MyEzMed may use and disclose your information for treatment, payment, healthcare operations, or when required by law. We will not use or share your information for other purposes without your written authorization. Psychotherapy notes are maintained separately from the medical record and receive special protection under federal law. MyEzMed does not release psychotherapy notes except as required by law. You may request access to your medical record, including diagnoses, treatment plans, medication records, and progress notes, as permitted by law. We do not sell your health information.
Your Rights
You have the right to access your medical record, request corrections, request confidential communications, request limits on certain disclosures, receive an accounting of disclosures, receive a paper copy of this notice, and file a complaint. You will not be penalized for exercising your rights.
Our Responsibilities
MyEzMed is required by law to protect your health information, provide this notice, and follow the terms described here. We may update this notice at any time, and the current version will be available upon request.
Patient Responsibilities
As a patient, you agree to provide accurate and complete information, participate in your treatment to the best of your ability, keep scheduled appointments or provide timely notice if you need to cancel, follow agreed-upon treatment plans, and meet financial obligations as required by your health plan.
