Register your child for OGCMA Youth Programs and provide required consents. Please read all policies and waivers in full before agreeing.
Welcome

We are blessed and excited that you have chosen the Ocean Grove Camp Meeting Association Youth Ministry Program. Many years, prayers, countless hours of planning/preparation from dedicated staff and volunteers, and donor contributions enable us to offer a high caliber of programs and activities that reflect a commitment to excellence, spiritual formation, and fun. We hope that all who take part will know and grow in Jesus and come to see Ocean Grove as a place where lasting friendships are made and faith development is encouraged and supported.
Pertinent Information

All parents or guardians of a child under 18 (and all participants who are 18 years of age) that wishes to participate in any Ocean Grove Camp Meeting Association program or activity must complete the following release form before his/her child (or they) may begin the program/activity. This participation agreement contains waiver & release of liability and indemnity to which you will be bound, and under which you will be waiving important legal rights. Do not sign this agreement before you read it.

I understand and agree that:
All youth are expected to be able to engage in events with kindness and appropriate behavior (i.e., no hitting, bullying, or abusive language, abuse of any kind, and no displays of anger toward others). We seek to foster and support healthy and Godly ongoing personal relationships between peers and staff during programming.

Participants must abide by all behavioral expectations and instructions during event programming. If your child fails to act in kind ways, the staff will call the parent/guardian, and the participant will need to be picked up immediately. Some circumstances may warrant a suspension from programming.
Special / Behavioral Needs

We are committed to creating a safe, welcoming, and positive experience for every participant. Each child is unique, and we want to understand how we can best support them while they are part of our programs.

If your child has any medical, developmental, behavioral, sensory, or emotional needs that would be helpful for our staff to know, please share those with us below. The more we understand, the better we can care for and support your child.

In some cases, if a child requires consistent one-on-one support to participate safely and successfully, we may ask that a parent or designated support person attend alongside them during activities. This helps ensure the best experience for your child and for the group as a whole.

Any designated support person must be 18 years of age or older and complete a volunteer form, including a background check (the cost of which will be reimbursed).

Waiver & Release Of Liability

Participation Agreement For Youth Recreation

All parents or guardians of a child under 18 (and every participant who is 18 years of age) that wishes to participate in any Ocean Grove Camp Meeting Association program or activity must complete the following release form before his/her child (or they) may begin the program/activity. This participation agreement contains waiver & release of liability and indemnity to which you will be bound, and under which you will be waiving important legal rights. Do not sign this agreement before you read it.
I do hereby assume full responsibility for any and all damages, injuries (including death), or losses that i and/or my child may sustain or incur, if any, while attending, practicing, participating in or witnessing any Ocean Grove Camp Meeting Association exercise program, sport or physical activity occurring in or about the Ocean Grove Camp Meeting Association’s premises or at any offsite location. I hereby assume full risk, waive all claims and release and hold the Ocean Grove Camp Meeting Association, its instructors, or partners of said program or event, individually or otherwise, harmless for any and all claims for injuries or damages.

I am fully aware and understand that the Ocean Grove Camp Meeting Association does not have on or about the premises, an employ or contract with any medical services, provisions for ordinary and/or emergency medical services.

In consideration of my and/or my child’s participation in and the use of the Ocean Grove Camp Meeting Association’s facilities, I hereby release and covenant not to sue the Camp Meeting Association, its owners, shareholders, directors, officers, employees, representatives, agents, and lessees from any and all claims resulting from any physical injury that may occur to myself and/or my child while participating in any program or event sponsored by the Ocean Grove Camp Meeting Association.

Photo and Video Release Form

I hereby authorize the Ocean Grove Camp Meeting Association to publish the photographs and videos taken of me and/or the undersigned minor children for use in any future OGCMA's printed publications, website and social media.

I release the OGCMA from any expectation of confidentiality for the undersigned minor children and myself and attest that I am the parent or legal guardian of the children listed below and that I have the authority to authorize the OGCMA to use their photographs, videos and names.

I acknowledge that since participation in publications and websites produced by the OGCMA is voluntary, neither the minor children nor I will receive financial compensation.

I further agree that participation in any publication and website produced by the OGCMA confers no rights of ownership whatsoever. I release OGCMA, its contractors and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.

I understand and agree that:
All participants will sign-in for the duration of each program. If someone needs to leave early, he or she will communicate this to the staff with a note, text, or phone call from the guardian and will sign-out before leaving early. If it is for work, the participant can show a staff member his or her work schedule necessitating the early leave, but must still sign-out before leaving.

OGCMA is not responsible for any participant aged twelve and older who leaves programming early without parental permission AND staff notification.
