Read/Write & Math Intervention programs provide structured, remedial and intervention-based programming. Please identify the concerns you have with your child's literacy and/or numeracy skills.
Program Selection
Please review the program information on our website prior to selecting your requested program(s). If you have questions about our programming, please contact our Intake Department prior to submitting your application. Program selections cannot be changed by email request after submission; a new application will be required.

Additional Program Information:
- Concurrent Booster and/or Intensive programming is not offered, in alignment with best practices in intervention.
- Blended programming is only offered for Intensive programs and is a departmental decision after a request is made. RWM assessment will determine if this is an appropriate intervention program decision.
- Requests for registration in multiple programs will be considered; however, placement is not guaranteed. Our priority is to ensure program access for as many families as possible.
- Instructor preferences or requests are not accepted. Student placement will be determined by our team based on individual learning needs, program structure, and staff expertise and availability. Applications that include instructor requests will not be processed and will require reapplication.

Health Forms are required and must be submitted with each new application. To ensure your application is processed, please ensure all requested information is correctly entered on the Health Form. Two (2) emergency contacts who are NOT listed as legal parent/guardian(s) are required. Applications received without Health Forms will require reapplication and will result in loss of placement in the registration queue.
Bursary Program
Bursary Application Form
Financial assistance is available through Foothills Academy. Bursary applications (including all supporting documents) must be submitted BEFORE submitting this program application. Program applications received without submission of bursary application will not be processed and will require a full reapplication. Would you like to apply for bursary?
Bursary applications are active for one year from date of application. If you have submitted a bursary application within a year, you do not need to submit a new bursary application. If you are unsure, please contact Dave Bolink, Director of Business Management, to confirm BEFORE you submit this program application.
