This Mentorship Enrollment Form is designed to help us prayerfully understand your current season, spiritual needs, and the direction the Lord is leading you. As you complete this form, we encourage you to be open and led by the Holy Spirit, knowing that every response will assist our leadership team in building a mentorship plan that aligns with God’s will for your life.

Subscription Terms & Commitment

All mentorship subscriptions are offered as a 6-month commitment, whether you choose to pay monthly or pay in full for six months in advance. By enrolling, you agree to this 6-month renewable term.

Please note that certain program benefits and opportunities may not be accessible if your subscription is canceled before completing the full 6-month commitment period.

At the end of each 6-month term, your subscription will automatically renew unless a cancellation request is submitted. To cancel or request changes to your subscription, please email info@wifeoftheword.org prior to your renewal date.

I understand that the information I provide will be used for assessment and program planning purposes and may be reviewed by the WOW Mentorship administrative team. I consent to this review and acknowledge that recommendations may be made based on the information submitted and my consultation session.
