Terms, Conditions, and Consent to Evaluate
By signing below, I, the parent/legal guardian, agree to the following:
1. Consent to Evaluate: I give permission for Elizabeth Greenwood of EverCurious Minds to administer the Naglieri General Ability Test (NGAT) and conduct a 1-on-1 Inquiry Task with my child.
2. Educational vs. Clinical Disclosure: I understand that this evaluation is for educational placement and consulting purposes only. It is not a clinical psychological assessment, a neuropsychological exam, or a medical diagnosis. I understand that Elizabeth Greenwood is an educator/consultant and not a licensed clinical psychologist.
3. Test Integrity: I understand that to protect the integrity of the assessment, raw test protocols (individual questions and answers) cannot be released. I will receive a Comprehensive Evaluation Summary including scores and instructional recommendations.
4. No Guarantee of Admission: I understand that the evaluation fee covers the cost of testing materials and professional time. It does not guarantee admission into an EverCurious Minds cohort, as placement is determined by the fit of the peer group and the student’s readiness for high-intensity inquiry.
5. Cancellation & Rescheduling: I agree to provide 48 hours' notice for any rescheduling needs. I understand that evaluation fees are non-refundable, as materials and time slots are reserved specifically for my child.
6. Confidentiality: I understand that all school records and test results will be kept confidential and will not be shared with third parties without my written consent.
