This training explains how to respond to workplace injuries, blood/body fluid exposures, chemical exposures, and safety incidents/near-misses. Prompt reporting is required. Completion is required before assignment.

SECTION 2 — Training Content
A) What counts as a safety incident
- Injury/illness at work; needlestick/cut; splash to eyes/mouth; chemical exposure; slip/trip/fall; equipment malfunction; burn/steam burn; near-miss; unsafe condition.
B) Immediate actions (general)
- Stop work if unsafe; move to a safe area; notify supervisor.
- Get first aid/medical help as needed.
C) Exposure response (examples)
- Blood/body fluid: wash with soap/water; flush eyes/mucous membranes; report immediately; seek occupational health evaluation.
- Chemical splash: rinse/flush immediately; use eyewash/shower; follow SDS and supervisor directions.
- Cut/puncture: wash; control bleeding; report and seek evaluation.
D) Incident/near-miss reporting
- Report immediately to onsite supervisor per facility policy.
- Complete facility incident report if required.
- Report to Charisma SPD same day (immediately for serious events).
E) Stop-work authority
- Staff may stop work when conditions are unsafe or PPE is missing/damaged; do not improvise.
F) Confidentiality & no retaliation
- Reports handled as confidentially as possible; retaliation for reporting is prohibited.

SECTION 3 — Knowledge Check
