Incomplete forms, or forms submitted without the required information, contact details or supporting evidence, cannot be progressed and may be returned.
Child Safeguarding
If you have concerns that a child or young person is at immediate risk of harm, please contact emergency services by dialling 000 without delay. If you have concerns about the safety, welfare or wellbeing of a child, you should report them directly to Child and Youth Protection Services in the ACT on 1300 556 729 or by email to childprotection@act.gov.au
Reports to child protection authorities are separate to this form and should not be delayed by completing this complaint form. Reporting to the appropriate authorities helps ensure a child’s safety is prioritised and responded to by the correct services.
Important – Club Resolution First
For operational, team, competition or club management matters, clubs must be given the opportunity to assess and resolve concerns in the first instance.
Participants are encouraged to raise these matters with their Club’s Member Protection Information Officer (MPIO) or relevant club official prior to escalation to Capital Football.
However, concerns relating to safeguarding or child safety may be reported directly to Capital Football and do not need to be raised with club beforehand.
Matters submitted to Capital Football that are more appropriately managed at club level may be redirected for initial review.
Clubs are expected to work collaboratively to resolve issues through clear and respectful communication.
Submission Options
1. Formal Complaint
A formal complaint must include the required information, sufficient detail, and the complainant’s contact details to enable assessment and progression. All formal complaints will be assessed in accordance with Capital Football’s policies and procedures.
Following assessment, Capital Football will determine the appropriate course of action, which may include no further action where the matter does not fall within its jurisdiction or does not warrant progression.
2. Incident Notification (Non-Formal)
If you do not have sufficient information to lodge a formal complaint, or wish to remain anonymous, you may submit an Incident Notification.
Incident Notifications are not treated as formal complaints and may not result in a formal investigation or specific outcome. Notifications are recorded for monitoring purposes and may be reviewed if further information becomes available or if similar reports are received.
Anonymous submissions can only be recorded as Incident Notifications and cannot be progressed as formal complaints.
