JAGUAR BASKETBALL CAMP — LIABILITY WAIVER & RELEASE
I, the parent/guardian of the participant listed on this form, give permission for my child to participate in Jaguar Basketball Camp held at Windsor High School Gym.
I understand that participation in athletic activities involves inherent risks, including but not limited to slips, falls, collisions, and other injuries, which may be serious. I knowingly and voluntarily assume all risks associated with participation.
In consideration of being allowed to participate, I hereby release, waive, and discharge Jaguar Basketball Camp, its coaches, volunteers, staff, and representatives from any and all claims, demands, causes of action, and liability arising out of or related to participation in camp activities, except in cases of gross negligence or willful misconduct.
Medical Authorization: In the event of injury or illness, I authorize camp staff to obtain emergency medical treatment for my child. I understand that I am responsible for any medical expenses incurred.
Photo/Video Release (Optional): I grant permission for photos/video of my child taken during camp to be used for camp/program communication and promotion (website, social media, flyers).
