I give permission for OTS Academy management and coaches to provide or seek medical treatment for my child/player in case of emergency.
Waivers & Agreements
I understand the risks associated with participation in this event and voluntarily assume full responsibility for any injuries and conditions that may arise. I am aware that OTS Academy is not liable for any injuries that may occur during participation in this event. As the parent/guardian of the participant, I have read and give my consent for participation.
I agree to release and hold harmless OTS Academy, its staff, and affiliates from any liability arising from food-related allergic reactions, except in cases of gross negligence.
I acknowledge that food will be provided during the event. While reasonable precautions will be taken, I understand that food may contain or come into contact with common allergens including, but not limited to, nuts, dairy, gluten, and eggs .
Are you the parent/guardian of the participant okay with photos or recordings being taken of the participant during the event that may be shared and posted on our social media
I agree that my child/player will follow the code of conduct and respect all staff, participants, and facilities.

Fee is $75 + HST = $84.75 FOR OTS ACADEMY ATHLETES

For All Other Athletes, Fee is $100.00 + HST = $113.00

We accept e-transfer to info.otsacademy@gmail.com, cash or payment by credit card.
Note: To pay by credit card, please reach out to otsacademy.admn@gmail.com There will be a 3.5% administrstion fee added to all credit card payments.

All registration fees are non-refundable.

Once payment is received, no refunds will be issued for any reason, including but not limited to illness, injury, scheduling conflicts, or withdrawal from the event.

In the event that the clinic is canceled by OTS Academy, participants will be offered either a rescheduled date or a refund.
