    Client Service Agreement

     
    1. CLIENT ACKNOWLEDGEMENTS

    By signing this Agreement, you acknowledge and agree to the following:

    • Dr. Woof’s Veterinary Care is a mobile veterinary practice and is not a full-service veterinary clinic. It is your responsibility to maintain a relationship with a veterinarian in a clinic or hospital setting for your pet’s primary and emergency care.
    • Dr. Woof’s Veterinary Care does not provide emergency or after-hours services. You are responsible for having a backup emergency veterinary provider at all times.
    • Treatment results are not guaranteed. Dr. Woof’s Veterinary Care makes no promises or guarantees of cure or outcome.
    • Due to the nature of mobile veterinary practice, the arrival time for scheduled appointments is within a thirty (30) minute window of the scheduled appointment time. Arrival times may be affected by factors including, but not limited to, traffic, weather conditions, and the duration of preceding appointments.
    • If surgery, anesthetic dental care, radiographs, ultrasound, or other imaging is needed, Dr. Holly Rice will refer you to trusted veterinary colleagues who provide those services.
    • Active client status requires at least one visit within the preceding twelve (12) months.

    2. HOURS & AVAILABILITY

    Business hours are Monday through Friday, 9:00 a.m. to 5:00 p.m., excluding weekends and major holidays. Hours are subject to change with reasonable notice. We endeavor to respond to all phone and email inquiries received during business hours within one (1) business day. Questions about a pet’s health between visits may be subject to a consulting fee or require scheduling a telemedicine session.

    3. TELEMEDICINE

    Telemedicine services (phone, video, text, or email consultations) are available only for follow-up of existing conditions where a Veterinarian-Client-Patient relationship has been established. Telemedicine is not a substitute for a physical examination and is not available for new conditions or pets not previously seen by Dr. Holly Rice.

    4. SERVICES & PRICING

    All services are provided at Dr. Woof's Veterinary Care's then-current pricing. A written estimate may be provided in advance of your appointment; however, if pricing has changed between the date of the estimate and the date of treatment, you will be provided with an updated estimate reflecting current pricing at the time of your visit. Pricing is subject to change at any time without prior notice. By proceeding with treatment after receiving an estimate, you agree to the fees set forth in that estimate.

    5. PAYMENT

    Payment is due in full at the time of service. Accepted payment methods include debit cards, credit cards (Visa, Mastercard, CareCredit), Zelle, ACH transfers, and cash. We do not accept physical checks, PayPal, Venmo, Apple Pay, Discover, or American Express, and do not carry change. Payment policies are subject to change.


    6. CANCELLATION POLICY

    A minimum of forty-eight (48) hours’ notice is required to cancel or reschedule any appointment. Our scheduling is route-based, and late cancellations directly affect our ability to serve other clients.

    Cancellations with less than 48 hours’ notice and missed appointments (“no-shows”) will incur a cancellation fee of $95.00. If you are not home at the time of your scheduled appointment, you are responsible for the full consultation and house call fees.

    Two or more late cancellations or no-shows within a twelve-month period will require a non-refundable deposit at the time of scheduling for all future appointments.

    7. CREDIT CARD ON FILE 

    A valid credit or debit card on file is required for all clients. Card information is stored securely through our encrypted, PCI-compliant payment processing system. Dr. Woof’s Veterinary Care does not have access to your full card number.

    By signing this Agreement, you authorize Dr. Woof’s Veterinary Care to charge the card on file for:

    1. Late cancellation fees as described in Section 6;
    2. No-show fees as described in Section 6;
    3. Any outstanding balance for services rendered.

    You will be notified before any charge is processed. It is your responsibility to keep your card information current. If the card on file is declined, you must provide an alternative payment method within five (5) business days. This authorization remains in effect for the duration of the client relationship and may be revoked only in writing. Revocation may result in the inability to schedule future appointments.

    8. PHARMACY

    Prescription medications and supplements may be available at the time of your appointment or shipped through our online pharmacy partners. Prescriptions can be electronically approved but cannot be called in or faxed to third-party pharmacies. In accordance with applicable pharmacy regulations, dispensed prescription medications cannot be accepted for return or refund once they have left our possession.

    9. VACCINE POLICY

    Arizona law (A.R.S. § 11-1010) requires all dogs three months of age or older to be vaccinated against rabies. As a policy of Dr. Woof’s Veterinary Care, we require proof of current rabies vaccination or administration of a rabies vaccine at the time of your pet's appointment, unless the pet is deemed too ill to receive the vaccine. Titers are not accepted in lieu of vaccination. If your pet has a history of severe vaccine reaction, we are unable to administer vaccines. Due to the mobile nature of our practice, we may not be able to return to treat an unexpected vaccine reaction.

    10. SAFETY

    We reserve the right to refuse or discontinue service if our safety is at risk. This includes, without limitation, the following:

    • Homes where cigarette or marijuana smoke is present or where smoking has recently or regularly occurred;
    • Homes where individuals are visibly impaired by alcohol or drugs;
    • Pets that require injectable sedation or are too fearful or aggressive to be safely handled.
    • If service is refused or discontinued due to safety concerns, consultation and house call fees still apply.

    11. RESPECT POLICY

    The veterinarian-client relationship thrives on mutual respect. Bullying, dishonesty, intimidation, or harassment in any form will not be tolerated. Violation of this policy by a client will result in dismissal as a client of Dr. Woof’s Veterinary Care and Dr. Woof’s Veterinary Care will release your pet’s medical records upon written request.

    12. TERM & MODIFICATIONS

    This Agreement is effective upon signing and remains in effect for the duration of the client relationship unless terminated by either party. Dr. Woof's Veterinary Care reserves the right to modify the terms of this Agreement at any time upon written notice to the Client. Continued use of the  services following receipt of such notice constitutes your acceptance of the modified terms.

    13. GOVERNING LAW & DISPUTE RESOLUTION

    This Agreement shall be governed by and construed in accordance with the laws of the State of Arizona, without regard to its conflicts of law provisions. Any legal action or proceeding arising out of or relating to this Agreement shall be brought exclusively in the state or federal courts located in Maricopa County, Arizona. Each party irrevocably consents to the personal jurisdiction of such courts and waives any objection to venue therein.

    In any action to enforce or interpret this Agreement, the prevailing party shall be entitled to recover reasonable attorney’s fees, costs, and disbursements in addition to any other relief granted.

    14. SEVERABILITY

    If any provision of this Agreement is found to be invalid, illegal, or unenforceable, the remaining provisions shall continue in full force and effect.

    15. ENTIRE AGREEMENT

    This Agreement, together with the separate Consent to Treatment and Assumption of Risk form, constitutes the entire agreement between the parties regarding the subject matter herein and supersedes all prior or contemporaneous agreements, understandings, or representations.
