  • LIABILITY RELEASE STATEMENT

  • My daughter/son has permission to attend the Stake Camp at Treasure Mountain sponsored by the Church of Jesus Christ of Latter-day Saints. I, the undersigned parent/guardian and I, the minor, do hereby authorize the Stake Leaders or any such substitute as may be designated as agent(s) for the undersigned to consent to any X-ray, examination, anesthetic, medical or surgical diagnosis or treatment and hospital care which is deemed advisable by, and is to be rendered under the general or special supervision of any physician and surgeon licensed under the provisions of the Medical Practice Act on the medical staff or any accredited hospital, whether such diagnosis or treatment is rendered at the office of said physician or said hospital; or any X-ray or examination, anesthetic, dental or surgical diagnosis or treatment, and hospital care to be rendered by a dentist licensed under the provisions of the Dental Practices Act.

    It is understood that this authorization is given in advance to any specific diagnosis, treatment, or hospital care being required but is given to provide authority and power on the part of our aforesaid agent(s) to give specific consent to act and all such diagnosis treatment or hospital care which the aforementioned physician or dentist in the exercise of his or her best judgment may deem advisable. This authorization is given in pursuant to the provisions of California Family Code Section 6910.

    I also authorize any hospital, which has provided treatment to the above-mentioned minor pursuant to California Family Code Section 6910, to surrender physical custody of such minor to my above named agent(s) upon completion of treatment. This authorization is given pursuant to the California Health and Safety Code 1283.

    I also authorize adult hike leaders to administer emergency medical treatment to the above-mentioned participant for any accident or illness and to act on my behalf in approving medical treatment that may be required. I give permission to the Stake Leaders and volunteer staff to arrange necessary transportation, provide routine health care, administer prescribed medications, and seek emergency medical treatment for the camp participant.

    This authorization will remain effective while the above minor is en route to or from, involved, or participating in any Stake Youth activity unless revoked in writing by the above, signed, and delivered to the aforesaid agent.

    I understand that the camper's participation in activities may involve exposure to hazards and potential risk of injury, including illness, hazards and risks related to travel to and from the location of the activities. I have had an opportunity to ask questions and to receive answers concerning those risks. I understand that my child is assumingall risks and hazards of loss or injury of any kind whatsoever that may arise in connection with the activity. I agree to release from liability and hold harmless the Stake I am attending with as well as the Church of Jesus Christ of Latter Day Saints, and its officers, leaders, and agents, for losses, costs, or damages arising from my daughter's/son's participation in such camp and its related activities, and agree to indemnify each of them from any and all claims, costs, suits, actions, judgments, and expenses, u pon any damage, loss or injury to my child or damage to my child's property arising out of my child's participation in this program.

    • I give permission for my daughter/son to appear in camp photographs and videos taken for non-commercial purposes and put on camp websites.

  • Laws and Regulations Relating to Organized Camps

    30750 Health Supervision
    (a) Every camper and each staff member entering camp shall furnish a health history of his or her health status that is completed and signed by the individual camper or staff member, or by the parent or guardian if the camper or staff member is under the age of 18. This history shall be kept on site as long as the camper or staff member is at camp and shall include the following:
    (1) A description of any health condition requiring medication, treatment, special restrictions or consideration while at camp.
    (2) A record of immunizations including date of last tetanus shot.
    (3) A record of any allergies.

  • Lincoln California Stake Standards of Conduct for Youth Activities and Camps (2026)

    There is a time and a place for many things. Youth camps and activities are wonderful opportunities for physical, social and spiritual enrichment for Church youth and nonmembers who are willing to abide by Church standards (See Luke 2:52). Youth activities, and in particular overnight Aaronic Priesthood and Girls camps, are designed as a place for youth to be separate from worldly influences and are intended to be uplifting and welcoming for all involved. To that end, the Lincoln California Stake provides the following general guidelines which reflect the tenants of both the Church Handbook (See Sections 10.2.1.3; 11.2.1.3) and the FSY Standards of Conduct. 

    As a general matter, participants at youth camps and activities are always expected to follow all principles taught in For the Strength of Youth: A Guide for Making Choices. The primary purpose underlying youth camps and activities is to strengthen our faith in Heavenly Father and Jesus Christ. As such, youth activities and camps are not the proper venue for engaging in romantic behavior, playing pranks, or having conversations regarding politics, unrevealed doctrine, or other sensitive topics. 

    Consistent with these principles, the activities listed below are inconsistent with Church teachings. Youth leaders are obligated to notify a youth’s parents and may, in some circumstances, send the youth home if found participating in one or more of the following: 

    ● Participating in or encouraging immoral behavior of any kind, which includes breaking the law of chastity or viewing or distributing pornography in any form 

    ● Theft or vandalism of any kind 

    ● Breaking the Word of Wisdom, including vaping and possessing illegal or harmful substances (e.g. marijuana, alcohol, tobacco products, etc.) 

    ● Possessing weapons, including firearms, explosives or similar items ● Harming or threatening to harm themselves or others physically, spiritually, or emotionally, including any form of bullying 

    ● Leaving camp without following proper check-out procedures, including skipping scheduled activities without permission and breaking curfew 

    Additionally, parents must make sure there is someone who is both locally available and legally responsible to transport, make medical decisions for, and otherwise care for your youth in case of an emergency situation. 

    When our youth gather together, they have the opportunity to create lasting gospel-centered relationships, make lifelong memories and build unity and faith. We appreciate efforts by our Stake’s parents and youth to comply with these standards.
