Preparing for your appointment:* This field is required.We require a deposit at the time of appointment confirmation. The deposit amount varies between artists. Deposits are NON REFUNDABLE, NON TRANSFERABLE, and will come off the final price of the tattoo. In the event of multiple sessions, your deposit will come off the final session. Cancellations under 48 hours, not show up, or arriving over 30 minutes late; can and will result in the loss of your deposit. Due to COVID-19, we are currently not allowing any guests. Please show up well rested and on time. Not every design can be finished in the allotted amount of time. Multiple sessions may be required. I understand, have read and completed this questionnaire truthfully. I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures.
