NEW GROUP FORM - Member Interest Groups

Thank you for your interest in setting up a member interest group within PQMD's member network. Member interest groups will be furnished with a Teams Channel and a suite of collaborative tools.

Filling out this form will launch a process for PQMD to consider the formalization of this interest group. Information on the process, PQMD standards and recommendations for leaders of interest groups is available through our guidance documents.
