• HJHS/HFA Knott's Berry Farm Field Trip Waiver and Medical Authorization

    Field Trip Details
  • On Friday, May 1, our eighth grade HJHS and HFA students will be going on a field trip to Knott's Berry Farm! The cost of the field trip is $50 per person, which includes transportation via charter bus and the entrance fee. Permission slips and payment must be made online no later than Wednesday, March 25. Due to payment deadlines with the venue, we cannot add reservations after the deadline.

    • We will board buses soon after school begins at 8:05 a.m. and are expected to return at approximately 6:30 p.m.
    • HJHS/HFA Free Dress will be strictly enforced. Any students not abiding by the guideline will not be permitted to attend. Please click here to review our Free Dress Policy.
    • Please leave all school backpacks at home. A small drawstring type bag for personal products is permitted. 
    • Students who bring cell phones will need to keep them put away and in their pockets for the day. Chaperones will allow students to use their cell phones to provide ETA to parents. 
    • Students will be outside walking all day; it is recommended that they wear sunscreen, comfortable walking shoes, and bring an empty or unopened bottle of water. 
    • All personal items brought on the field trip are the student’s responsibility. Any lost or damaged items are not the responsibility of the school or venue. 
    • OUTSIDE FOOD AND BEVERAGE IS NOT PERMITTED. Knott’s Berry Farm is CASHLESS. Visit knotts.com/cashless for more information. There are Kiosks to convert cash to a prepaid card (we’ve heard these lines can be very long) to use at the park or students can use Apple or Google pay. Parents can also go online to purchase meal packages for the day. Visit https://www.knotts.com/dining/drinks-dining-deals for more information.
    • There are no refunds for students who do not attend on the day of the field trip.

    I hereby give my permission for my child to participate in a field trip to Knott's Berry Farm located at 8039 Beach Boulevard, Buena Park, CA 90620.

    I fully understand that my child is to abide by all rules and regulations governing conduct during the field trip. It is understood that any child determined to be in violation of these behavior standards may be sent home at the parent or guardians’ expense. I understand and acknowledge that consenting to allow my child to participate in this field trip, I shall, by law be deemed to have given up all claims against American Heritage Charter Schools and each of its officers, employees and agents (hereinafter referred to as AHCS) for any injury, accident, illness or death occurring during or by reason of the field trip. I also agree to relieve AHCS of responsibility for damage to or loss of my child’s property during or by reason of the field trip. In the event of any illness or injury, I hereby consent to whatever x-ray, examination, anesthetic, medical, dental or surgical diagnosis or treatment and hospital care from a licensed physician and/or surgeon as deemed necessary for the safety and welfare of my child. It is understood that the resulting expenses will be the responsibility of the parent(s), guardian(s), or participant. Whenever possible, attempts will be made to contact the parent/guardian prior to any medical action.

    -Participation in the Knott's Berry Farm field trip is a privilege and administration may prohibit students from participating at any time if the student is not meeting academic expectations or has any discipline incidents administration deems disqualifying.

    -Student dress is in accordance with the FREE-DRESS dress code.
