Authorization to Verify Information
I authorize Quemado Independent School District and its designated representatives to verify information contained in this application, including employment history, education history, licensure status, and professional references, for the purpose of evaluating my qualifications for employment.
I authorize former employers, educational institutions, licensing agencies, and other relevant entities to provide information requested by the District consistent with New Mexico law. A photocopy or electronic copy of this authorization shall be as valid as the original.
Pursuant to NMSA 50-12-1, I understand that employers providing information regarding my prior employment are granted immunity from civil liability when such information is provided in good faith.
Background Investigation & Fingerprinting
I understand that, if selected as a finalist or offered employment, I will be required to submit to a criminal background investigation, including mandatory fingerprinting, as required by New Mexico law and regulations of the New Mexico Public Education Department.
I understand that employment is contingent upon satisfactory completion of all required background checks.
In accordance with the New Mexico Criminal Offender Employment Act (NMSA 1978, Section 28-2-1 et seq.), a criminal conviction will not automatically disqualify me from employment; however, the District may consider convictions that are directly related to the position sought and consistent with statutory requirements governing school employees.
Educator Misconduct & Reporting (HB 128 Compliance)
I understand that under New Mexico law, including the requirements strengthened under House Bill 128 (2023), school districts are required to report certain educator misconduct, ethical violations, or licensure concerns to the New Mexico Public Education Department.
If employed, I understand that any substantiated findings of misconduct may be reported to the New Mexico Public Education Department in accordance with state law.

Information obtained through this application process will be used solely for employment consideration and will be maintained in accordance with applicable state and federal confidentiality laws. Information may be disclosed as required by law, court order, or lawful public records request consistent with the New Mexico Inspection of Public Records Act (IPRA).
