I hereby certify that the player is medically fit to participate and I agree that, without prejudice to the rights at common law of the designated parties, I indemnify Brooklyn Hockey Club NPC, all school's with Astros we use, NBHA, Boithabiso Sport NPC, SAHA or any of it's workers for any loss, liability, damage or expense which the player might suffer as a result of or which may be attributable to any act and/or omission of one or more of the designated parties and/or as a result of any injury at the sport, even if caused by perceived negligence. Please note that you will be liable for applicable fees on completion and submission of this form. The party who breaches a term of this agreement (the party in mora) shall be given 7 days written notice per WhatsApp, email or letter to rectify such breach. If the party in mora fails to rectify the said breach (despite such notice) within a period of 7 days of receiving the notice, then the notifying party shall have the right to claim specific performance in the event to claim damages and all legal costs of the party in mora. I further agree to pay the monies due as per my subscription above. I understand that, any legal costs to recoup outstanding money will be for my account.
