GREGORY FAMILY FARM EVENTS AND EDUCATION, LLC HOLD HARMLESS AGREEMENT

GREGORY FAMILY FARM EVENTS AND EDUCATION, LLC Agreement to Hold Harmless, assume risk, and release from liability I accept all terms as a participant at Gregory Family Farm Events and Education, LLC/Gregory Family Farm/Gregory Farms/Farm School, held at 611/611B Dobbins Pike, Gallatin, TN 37066. I agree to assume all risks associated with this activity and enter the following Agreement: I understand and agree that I have responsibility to exercise due care for my own safety while present on or about the farm and property. I acknowledge, appreciate and agree that the risk of injury from farm related activities (as well as activities near large crowds, temporary structures and large equipment) have the potential for rashes, stings, lacerations, bruising, fractures, permanent disability and death. I understand that there may be unsafe conditions on the property, including but not limited to partially-fallen trees in wooded areas, uneven ground, activities with live animals and nature. I, on behalf of myself, my personal representative, heirs, next-of-kin, minor dependents, spouse, and assigns, RELEASE AND FOREVER HOLD HARMLESS the following Released Parties: Gregory Family Farm Events and Education, LLC/Gregory Family Farm/Gregory Farms/Farm School, together with their respective members, owners, officers, representatives, managers, agents and assigns. I release and forever hold harmless the Released Parties from all claims, suits, causes of action, damages, demands, liabilities, duties and obligations of every kind, nature and description, known or unknown, arising out of my attendance to Gregory Family Farm Events and Education, LLC/Gregory Family Farm/Gregory Farms/Farm School. I knowingly and freely assume all risks, known and unknown, relating to these activities, even if arising another’s negligence, or from dangerous natural conditions of which the Released Parties are aware. While I intend to fully release the Release Parties, I agree in advance that the factual circumstances surrounding my activities, at a maximum, support the conclusion that I am a “licensee” for the purposes of premises liability. I assume full responsibility for any injury that I suffer as a result of event activities. I understand and agree that this Agreement covers every subsequent instance of vending or visiting the farm and property. I have read this Agreement, fully understand its terms, and understand that I have given up substantial legal rights by attending the farm. As a participant, you are giving up certain legal rights, including the right to recover damages in case of injury, death or property damage.

Medical Release and Authorization

As the parent and/or legal guardian of the named minor child, I authorize Gregory Family Farm and its affiliates, including staff and volunteers, to obtain emergency medical care for my child if I cannot be reached.

I authorize a licensed medical professional to examine, diagnose, and treat my child in the event of an emergency that, in the opinion of the medical provider, requires immediate attention to prevent further injury, illness, or harm.

This authorization includes permission for emergency treatment, diagnostic procedures (including X-rays), and minor medical or surgical treatment as deemed necessary by the attending medical professional.

I understand that every reasonable effort will be made to contact me before treatment when possible.

This authorization is given for the dates and duration of my child’s participation in activities at Gregory Family Farm.

I give this authorization freely and for the purpose of ensuring the health and safety of my child in the event of an emergency.
