2026 Youth Camp Waiver and Release Language for Jotform Registration form

In consideration of my minor child/ward (the “Participant”) being allowed to participate in ILIAD ATHLETICS, LLC’s (“Illiad Athletics”) programs, related events, and activities (“Camp”), the Participant and myself, acknowledge, appreciate, and agree to the following:

ACKNOWLEDGEMENT AND ASSUMPTION OF RISKS

I ACKNOWLEDGE AND UNDERSTAND THERE ARE INHERENT RISKS—INCLUDING THE RISK OF SERIOUS INJURY AND DEATH—ASSOCIATED WITH OUTDOOR WILDERNESS CAMP ACTIVITIES, including but not limited to: exposure to the elements and nature in all of its forms, performance of rigorous physical activity, use of bladed instruments, fire building, and other activities designed to challenge and develop campers physically, mentally, and emotionally. Camp activities have been described to my satisfaction. FOR MYSELF, MY SPOUSE, AND THE PARTICIPANT, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS ASSOCIATED WITH ILIAD ATHLETIC’S CAMP ACTIVITIES, BOTH KNOWN AND UNKNOWN TO ME AT THIS TIME, EVEN IF ARISING FROM THE NEGLIGENCE OF ILIAD ATHLETICS OR OTHERS.

LIABILITY WAIVER AND RELEASE OF CLAIMS

ON BEHALF OF MYSELF AND THE PARTICIPANT, I RELEASE, DISCHARGE, AND AGREE TO HOLD HARMLESS THE CAMP SITE PROPERTY OWNER(S) AND ILIAD ATHLETICS, INCLUDING THEIR OWNERS, EMPLOYEES, VOLUNTEERS, CONTRACTORS, AFFILIATES, AGENTS AND TRUSTEES, FROM ALL LIABILITY, CLAIMS, AND CAUSES OF ACTION OF ANY KIND, FOR INJURY, INCLUDING DEATH, DISABILITY, OR ILLNESS, OR LOSS OF PERSONAL PROPERTY OF THE ABOVE NAMED PARTICIPANT RELATED TO OR ARISING FROM CAMP ACTIVITIES, INCLUDING TRANSPORTION SPONSORED BY OR RELATED TO CAMP ACTIVITIES, EVEN IF ARISING FROM THE NEGLIGENCE OF THE CAMP SITE OWNER(S) OR ILIAD ATLETICS. THIS RELEASE IS ALSO INTENDED TO INCLUDE ALL LIABILITY, CLAIMS, AND CAUSES OF ACTION OF ANY KIND MADE BY THE PARTICIPANT’S FAMILY, ESTATE, HEIRS, PERSONAL REPRESENTATIVE OR ASSIGNS.

DUTY TO DEFEND AND INDEMNIFY

I FURTHER AGREE TO DEFEND, INDEMNIFY AND HOLD THE CAMP SITE PROPERTY OWNER(S) AND ILIAD ATHLETICS HARMLESS FROM ANY LIABILITY, CLAIMS, AWARD, AND CAUSES OF ACTION OF ANY KIND, ASSERTED AGAINST EITHER, INCLUDING THEIR OWNERS, EMPLOYEES, VOLUNTEERS, CONTRACTORS, AFFILIATES, AGENTS OR TRUSTEES, RELATED TO OR ARISING FROM CAMP ACTIVITIES, INCLUDING CAMP SPONSORED TRAVEL OR TRAVEL TO AND FROM CAMP. I, THE UNDERSIGNED, AGREE THAT MY DUTY TO DEFEND AND INDEMNIFY AS STATED IN THIS PARAGRAPH APPLIES IN THE EVENT THAT ANY LIABILITY, CLAIM, CAUSE OF ACTION, OR AWARD IS ASSERTED, SOUGHT, OR OBTAINED BY THE PARTICIPANT, THE PARTICIPANT’S FAMILY, ESTATE, HEIRS, PERSONAL REPRESENTATIVE OR ASSIGNS, RELATED TO OR ARISING OUT OF PARTICIPATION IN ILIAD ATLETICS CAMP ACTIVITY.

PHOTO RELEASE

I hereby grant ILIAD ATHLETICS the right to use, reproduce, and/or distribute photographs, films, videotapes, and sound recordings of the Participant, without compensation or approval rights, for use in materials created for the purposes of promoting and advertising the activities of ILIAD ATHLETICS.

CONSENT TO SEARCH

I hereby grant ILIAD ATLETICS the right to search the Participant’s belongings with the Participant present when, in the sole judgment of ILIAD ATHLETICS staff, the health, well-being, or safety of the Participant or others warrants it.

CONSENT TO TRAVEL

I hereby grant permission for the Participant to participate in all special, routine, or necessary Camp related trips off of the Camp site property with proper Iliad Athletics staff supervision.

As part of our safety requirements, parents must confirm that, in their honest judgment, their camper is mature enough to handle a knife and fire-making tools with adult supervision. This question must be answered “Yes” in order to proceed with registration.
