A USTA League Team Confirmation & Agreement form is required for all USTA League teams. Captains need to complete the information listed below and sign the form. Captains must sign the form for the team to be included in the match schedule. Captains need to acquire confirmation (verbal, email, or written) from your facility prior to submitting this Agreement.

Completed Team Confirmation Forms must be submitted before midnight on April 21, 2026. League play schedule will be published April 25, 2026.

A team number will be sent to the Team Captain after the Facility has also confirmed the team.
