PARTICIPATION AGREEMENT

The parent(s)/legal guardian of the above-named Participant(s) authorizes Abundant Life Church to act on his/her behalf when he/she cannot be reached, IN CASES OF EMERGENCY, resulting in the need for immediate medical attention for said Participant(s).

The parent(s)/legal guardian of the Participant(s) do hereby acknowledge that participation in the activity named above (the "Activity") involves risk to the participant and his/her parents/legal guardian, which may result in various types of injury.

Further, the Participant's parent/guardian releases and promises to indemnify, defend, and hold harmless Abundant Life Church, its agents, employees, volunteers, or any other representatives (collectively referred to hereinafter as the “Activity Sponsor”) for any injury arising directly or indirectly out of the described Activity, whether such injury arises out of the negligence of the Activity Sponsor, the Participant, or otherwise.

Further, the Participant's parent/guardian gives permission to the Activity Sponsor to use any photo or video taken of the Participant(s) to be used to illustrate, report, promote, or advertise the ministries of Abundant Life Church.

The Participant's(s') parent/guardian acknowledges that this form is being submitted electronically and attests that the signature affixed here is that of the person named as the Participant's(s') parent/guardian entered above. The signature on this form transmitted in electronic format shall be treated as an original signature for all purposes. The Participant's parent/guardian understands that this electronic form is legally binding as if submitted as a physically signed original.
