    Please submit one form per school so that we have all of your school's information in the one spot to make coordination simple. Participation is on a first in best dressed basis. There is no cost involved. Please note, all music and themes are screened for appropriateness for our particular event.

  • I have read the Info Pack and will pass pages 7 & 8 onto our Dance Teachers*
  • I will complete Form 2, the online Safety Briefing (when it is sent) and share all relevant info with our school community/dance teachers as per the Info Pack*
  • I understand that as a Dance Coordinator/Supervising teacher at my school, I am volunteering my time to supervise the students at my school/in my dance group at these concerts. Renumeration is not provided by Combined Schools Concert.*
  • Our Principal has given approval for us to participate in the Combined Schools Concert and acknowledges that all staff and parent/community members attending the event have the relevant WWCC and mandatory training required for supervision of students at the concerts.*
  • I understand that, upon completing this application form, withdrawing causes major disruption to the planning of the event.*
  • I will send the Publicity and Consent Form to our school dance community (this will be sent to you with your 'Acceptance Email'.*
