A $100 deposit must be submitted at the end of this form to secure your class date. If the class is not approved, your deposit will be fully refunded.

Range fees are not included.
The remaining balance is due by Day 2 of training.
All classes must be booked and the deposit submitted at least 7 days prior to the start date.
50% Deposit Required:

The 50% deposit due is $400, and must be submitted at the end of this form to secure your class date. If the class is not approved, your deposit will be refunded.

- Submitting this form does not guarantee approval. You are required to propose your preferred dates and submit payment before a request can be submitted to DCJS on your behalf.
- At the end of this form, you will receive a confirmation page with instructions to submit your deposit.
- If your requested dates are unavailable or are not approved by DCJS, you will have the option to select alternate dates for resubmission or receive your deposit back.
