Additional Comments:

• No Skating July 1st & August 3rd, 2026.
• From June 29th to July 31st sessions are offered at Hespeler Arena.
• From August 4th to August 21st sessions are offered at SportsPark Arena.
• Confirmations will be sent out approximately 1 week before we start.
• Skaters at the StarSkate level can purchase individual days, however there will be a 5 % surcharge. To find out the fee please contact the office.
• The Peppler Event is being held between approximately 4:30pm and 7:30pm on Friday July 17th, 2026.
• Private Lessons are billed separately.  Please contact your Base Coach to discuss lesson arrangements.
• All participants are required to have a home club.
• Registration form due by May 20th, 2026 to receive the discounted fees.
• Fees:  50 % of the fees are due when registering.  The next 25% is due on June 2nd and the balance is due July 2nd, 2026.
• After May 20th, 2026 the fees will be increased by 5%.  These fees will be adjusted and applied to your 2nd & 3rd payments.
• Registration is on a first come first serve basis.  
• Once the form has been submitted there are no refunds.

Informed Consent and Acknowledgement

I, the undersigned accept full financial responsibility for this application form.  I understand that there will be no refunds of deposits or due balances after the deadline dates.  I agree that Huston, Wheeler & Wheeler Skating Schools (KLFSS Inc) will not be held responsible for any accidents or loss, however caused and agree to release indemnity and save harmless the Huston, Wheeler & Wheeler Skating Schools (KLFSS Inc) from all damage or claims as a result of such accidents or loss.   I agree my child / ward will abide by the rules and regulations of the Champions Training Centre of Cambridge {KLFSS Inc} as set by the directors.  Any disregard for these rules will result in the removal of the Skater from our skating program without refund of any fees paid.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
