Electronic Signature Agreement
By selecting "I understand the above terms and conditions" below, you give consent for your son/daughter to attend the 2026 SFD Summer Camp(s) and understand that St. Francis DeSales High School, the Athletic Department, the Camp Director, and anyone connected with the Camp will assume no responsibility for accidents, or medical/dental expenses incurred as a result of attending any camp session.

By selecting "I understand the above terms and conditions" below, St. Francis DeSales High School (SFD) reserves the right to use any photograph/video taken at any summer camp sponsored by SFD, without the expressed written permission of those included within the photograph/video. SFD may use the photograph/video in publications or other media material produced, used or contracted by SFD including but not limited to: brochures, invitations, newspapers, magazines, websites, etc. To ensure the privacy of individuals and children, images will not be identified using full names or personal identifying information without written approval from the photographed subject, parent or legal guardian.

By selecting "I understand the above terms and conditions" below, you are signing this Agreement electronically. You agree your electronic signature is the legal equivalent of your manual signature on this Agreement. By selecting "I understand the above terms and conditions" you consent to be legally bound by this Agreement's terms and conditions. You also agree that no certification authority or other third party verification is necessary to validate your E-Signature and that the lack of such certification or third party verification will not in any way affect the enforceability of your E-Signature. You also represent that you are authorized to enter into this Agreement for all persons attending a summer camp at St. Francis DeSales High School.
