Sanction Application

This form is only for Sanctioned Meets. APPROVED MEETS must use the long form application
This form is for requesting a Sanction from Wisconsin Swimming, Inc. for an intra-squad, dual/tri/quad, or multi-session invitational, open or closed meet.

Only one application per form allowed.
If you have to submit more than one meet to be sanctioned, you will be redirected to add another application when complete.

APPROVED MEETS MUST USE THE

STANDARD APPROVAL APPLICATION

You will still be required to submit the Meet Recon before competition begins and the Meet Financial and Meet Report after competition concludes. The email, with the SANCTION NUMBER, will have further details on reporting requirements.

Only sanctioned meets may be submitted thru this process. As part of the sanction process clubs attest to following USA Swimming rules, MAAPP, Safe Sport 360, and applicable policy as noted at the end of the application. Please include the language from page 3 of the SafeSport 360 document in your meet packet, or it will be returned. This is in addition to any MAAPP policy language.
- The USAS Member ID's for the Meet Director, Referee and Admin Official are needed to complete this form. Credentials will be verified. If the member's credentials are not valid, a sanction may be terminated.
- Please provide contact information for the person submitting this application. THE PERSON SUBMITTING THE APPLICATION WILL BE THE PERSON WSI COMMUNICATES WITH
- Reference the Meet Operations Info sheet for any questions regarding assistants in position
