  • Permission Section

My Child has permission to take part in all Day Camp activities led by the United Methodist Church of Eagle Valley staff and volunteers. We agree that the Camp, Church, and their personnel will not be held responsible for accidents arising therefrom. This includes all walking activities, including trips to the Eagle Town Park and to Vail on public transportation and as described above. No children will be taken anywhere by vehicle unless in a medical emergency. I give Camp and Church personnel permission to seek medical treatment for my child in case of injury or illness. I also give permission for photos, video, and electronic images to be taken of me or my child and used by the Camp or Church for promotional purposes without compensation, inspection or approval.

  • If medical attention, beyond the superficial needs of minor accidents/injuries, is deemed necessary by the camp administration, the first course of action would be to determine the severity of the injury/illness. If an emergency, 911 will be called to initiate medical attention. The next level of care would be to call the parents/emergency contacts for them to come and pick up the child to attend to the medical needs. If these persons cannot be reached, the camp administrators will transport the child to Urgent Care in Eagle at the Eagle Medical Center. Eagle Urgent Care services are located at Colorado Mountain Medical in Eagle, CO, offering same day and walk-in appointments for patients with acute injury or illness, as a convenient and affordable alternative to the Emergency Department for non-life threatening medical issues, located on the lower level entrance, 0377 Sylvan Lake Rd  #120, Eagle, CO. This facility is open 8am-6pm each day of our camp, 970-926-6340. Parents will be fully responsible for all financial obligations regarding the above mentioned responses to medical needs. If a child has an Epi Pen or other medication needed in case of a known potential emergency, that medication must be in the possession of the camp counselor during day camp for immediate access. First Aid kits will be available to all counselors and camp staff.
All injuries will be reported to the parents/pick up contacts at the end of each camp day.

  • My child has permission to participate in all camp activities, except as noted. I hereby give permission to the medical personnel selected by the camp director to order X-rays, routine tests and treatment for the health of my child. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the camp director to hospitalize or secure proper treatment (including surgery, injection, and/or anesthesia) for my child/children as named above.
