I give consent
The parent hereby gives consent for the tutor to supervise their child and engage in the activities given in Schedule 1 at all times, taking reasonable care and precautions to ensure the child’s safety and wellbeing. The activities detailed in Schedule 1 and the risks associated with them are indicative and not an exhaustive list but are intended to give a reasonable representation of what is envisioned by the tutor and the kinds of risks that may be associated with those activities.
I understand and acknowledge the below
The parent hereby understands and acknowledges that whilst the tutor will exercise reasonable effort and care to encourage and, where practical, ensure the safety of the child’s property, the tutor cannot be held liable for the loss or damage of the child’s property other than where it arises from the tutor’s wilful misconduct or gross negligence. It shall always be the responsibility of the parent(s) to properly equip their child and insure their possessions against loss or damage, where appropriate.

The tutor will collect and store personal information relating to the child(ren) that they teach, including but not limited to teaching notes, records of attendance, assessment data and administrative records in order to facilitate their effective teaching, the administration of tuition and to deliver an effective service. This may include written records, digital records, photographs and video recordings. All personally identifiable information will be held privately and in confidence, in accordance with the tutor’s privacy policy. The parent hereby accepts and acknowledges that the tutor will collect and store such information, taking reasonable measures to prevent its unauthorised disclosure.

By signing below, you acknowledge that you have read and understood the information provided and consent to your child participating in the described activities in Schedule 1. You understand and accept that there are always risks involved in any activity and that whilst the tutor will endeavour to reduce and minimise such risks they cannot be eliminated entirely. You agree that you will assist the tutor and take responsibility to ensure that you are aware of the arrangements being made and to properly equip and prepare your child(ren) in line with the tutor's instructions.
