  • Sustainable Communities Expression of Interest:
    Thank you for your interest in the Sustainable Communities Initiative. The Expression of Interest (EOI) form is an initial way for us to learn more about your community, begin to understand what is occurring, and start a conversation about how we might work together.

  • Submission Timeline

    • EOIs are being accepted until April 20, 2026.
    • All submitted EOIs will be reviewed following the submission deadline.
    • We will follow up with all communities that have submitted an EOI at the end of April 2026.
    • Submitting an EOI does not represent a commitment, nor does it guarantee selection.
  • A Note on Invitations

    • A formal letter of request inviting RCCbc to your community is not required to submit an EOI. However, if your community is invited to move forward in the process, we will request a signed letter from community partners, providers, and your Health Authority inviting RCCbc into your community. We will work with selected communities through this process.
  • Completing the EOI Form

  • Please complete all sections of the form to the best of your ability. The form collects information in the following areas:
    • Community or region details
    • Primary applicant and contact information
    • Overview of the community landscape
    • Current primary care services (e.g., physicians, nurse practitioners)
    • Availability of primary care clinics and hospitals or afterhours access to care
    • Access to allied health services
    • Relevant community or regional initiatives
    • Potential partners and collaborators
    • Additional contacts and any other information you wish to share

    • What does primary care look like in the community such as how many physicians and/or Nurse Pracitioners? 
    • Is there a Primary Care clinic? 
    • Is there a hospital and if not is there access to afterhours care such as a diagnostic treatment centre?
    • Where is the nearest hospital? 
    • What does nursing support look like? 
    • Are there allied health professionals in the community such as Registered Midwives or Social workers?
