Please fill out the form in its entirety. Please be deposit ready. A non-refundable $100 deposit is required to secure the preferred date you wish to book. YOU MUST BE 18 OR OLDER. Please have your ID available the day of the appointment. Thank you!
My rates are as follows:
> Half Day, 12 PM - 4 PM: $400
> Full Day, 12 PM - 8 PM: $800
> Flash designs will come with a flat rate (It will not exceed my half day rate)
After you have filled out the form, I will contact you using your response preference with how the deposit may be paid.
RESCHEDULES: If you schedule the new appointment within the same month of the original appointment date no new deposit is required; however, rescheduling outside the month of the original appointment date will require a new deposit to be paid.
A new appointment date must be agreed upon within a week of request. If not, the appointment is considered canceled and a new deposit is required to re-book.
No call, no shows will forfeit the right to book with me in the future.
