PJC Baseball Camp Registration Form

Come join the PJC baseball team for our 2026 Youth Skills Camp held on March 12th at Hub Hollis Field! Check-In will begin at 8:30 AM with camp from 9:00-12:00. The cost is $60 (cash or check) paid at check-in. Your registration fee includes a camp t-shirt if registered by noon on March 9th. Informed Consent and Acknowledgement

I hereby provide my consent for my child's participation in all activities organized by the Paris Junior College baseball camp during the chosen camp session. By accepting my child's participation, I acknowledge and accept all the potential risks and hazards associated with these activities. I release, exempt, and indemnify Paris Junior College and all its officials, representatives, and agents from any responsibility for injuries that may occur to my child during their travel to, engagement in, or return from the camp sessions.

If my child sustains an injury, I waive any claims against Paris Junior College and the baseball camp, including its coaches, affiliates, fellow participants, supporting organizations, advertisers, and, if applicable, the owners and landlords of the premises where the event is held. I understand that this waiver and release may be stored electronically and agree that a copy is authentic and admissable as evidence in any future disput or proceeding.

Paris Junior College is requesting permission to use my name, likeness and/or biographical information that may be used in whole or in part for educational and non-sponsored publications or broadcasts for instruction or use as general information.

I hereby grant PJC permission to use my name, likeness and/or biographical information in any and all media, now known or hereafter developed, throughout the world in perpetuity, in connection with any and all versions of publications, video, audio or electronic presentations. I understand that I will not receive any compensation as a result of any use of my name, likeness, and biographical information as described in this release.

I waive any rights of privacy, and/or approval that I might otherwise have with regard to any promotional materials that might be published or broadcast.
