Acknowledgment & Assumption of Risk
I acknowledge that I am receiving spray tanning services provided by Golden Hour Beauty. I understand that spray tanning involves the application of sunless tanning solutions and that individual results may vary. I understand that solutions may contain DHA (dihydroxyacetone) and other cosmetic ingredients that may cause allergic reactions, skin irritation, dryness, or discoloration.
I confirm that I have disclosed any known allergies, sensitivities, medical conditions, pregnancy, or medications prior to service. Golden Hour Beauty is not responsible for reactions due to skin conditions or improper preparation or aftercare.
Release of Liability
I hereby release, waive, and discharge Golden Hour Beauty, its owners, employees, contractors, and affiliates from any and all claims, liability, or damages arising out of or related to spray tanning services, including but not limited to skin reactions,
uneven results, clothing staining, or dissatisfaction with cosmetic outcome. I understand that no specific results are guaranteed.
Aftercare Responsibility
I acknowledge that aftercare instructions have been provided or made available and understand that failure to follow them may affect results.
Consent
I voluntarily consent to receive spray tanning services from Golden Hour Beauty. I certify that I am at least 18 years of age, or that a parent or legal guardian has signed below. *Minors Under the age of 18 must wear bottoms*. This waiver applies to all current and future services unless revoked in writing.
