An updated version of this form must be signed and on file at all times for my pet to be admitted for services at Howling Woods K9 LLC. Howling Woods K9, hereby referred to as "HWK9" may provide updates or changes to its policies at any time.

GENERAL- HWK9 agrees to provide services to my pet as indicated on the HWK9 Dog Boarding Contract. HWK9 reserves the right to exercise its best judgement in all circumstances as to how to best provide the services selected and will exercise reasonable care to ensure the safety of my pet. HWK9 will keep its premises sanitary, properly enclosed, and ensure all pets are fed regularly and housed in clean and safe quarters.

PET HEALTH REQUIREMENTS- For the health and safety of all pets at HWK9, the following health prociedures are required:

IMMUNIZATIONS- All pets boarding with HWK9 are required to have the following immunizations performed by a certified & licensed veterinarian.

• Rabies (Puppies 4 months and younger excluded)
• Distemper
• Bordetella
• Canine influenza (highly recommended)

FLEA & TICK- All pets are required to be current on a monthly Flea & Tick preventative. If my pet is found to have parasites I will be notified immediately to come pick up my pet. If I do not pick up my pet within the same day, I grant HWK9 to provide any necessary treatment at my expense. If I provide these preventative medications with my pet upon drop off, HWK9 will administer on the date specified by me.

HEALTH & BEHAVIOR- HWK9 is equipped to care for older pets, administer certain medications with chronic conditions, however, they cannot care for the critically ill, aggressive or biting pets. HWK9 reserves the right to refuse my pet if, upon check-in, said pet appears to be sick or its behavior could jeopardize the health and safety of others. In the unexpected circumstance my pet requires hospitalization, HWK9 will provide transportation at my expense.

PAYMENT FOR SERVICE- I agree to pay for all charges quoted on my HWK9 Dog Boarding Contract in full at time of pick up. Charges begin the day I drop off my pet. If I fail to pay the remaining balance upon departure, HWK9 reserves the right to bill me and refuse all future reservation requests until my bill is paid in full.

CANCELATIONS- Notice of cancelation must be given 36 hours prior to the requested drop off date indicated on the HWK9 Dog Boarding Contract. If cancelation occurs after this time a full day of service will incur.

FIRST AIDE AND EMERGENCIES- I agree that HWK9 in its sole discretion, may be authorized to perform first aide, or other attention as deemed necessary for the health, safety, and wellbeing of my pet. If they believe that my

pet needs emergency veterinarian care, I authorize HWK9 to provide transportation to a veterinarian clinic. I will be notified by HWK9 of any emergency situations, prior to transporting my pet. In the event they cannot get in contact with me or my listed emergency contact, I authorize HWK9 to release my pet to a veterinarian clinic at my expense.

EMERGENCY CONTACT: In the event of an Emergency, HWK9 will do everything they can to contact me. If they cannot get in contact with me, an Emergency Contact will be notified. This person will be a designated agent, and is authorized to be used in place of myself.

PETS NOT PICKED UP ON DEPARTURE DATE: In the event myself or my agent do not pick up my pet on the scheduled departure date agreed upon at check in, HWK9 reserves the right to continue to provide the daily services set forth on the HWK9 Dog Boarding Contract for 5 days. After this my pet will be deemed "Abandoned" and the Abandoned Pet Procedures, as outlined below, will be followed.

ABANDONED PET PROCEDURE: Once deemed abandoned, all services for the pet will be terminated. HWK9 will send a certified letter (return receipt requested) to my address provided on this document the fifth (5th) day following the scheduled Departure Date, advising me that if my pet is not picked up within 10 days of the notification date, it will be deemed "Abandoned", and therefore HWK9 must contact the appropriate authorities for delivery or, where permitted by law, a third party adoption partner and placed for adoption. All fees associated herein, will be the responsibility of myself. I shall remain liable to HWK9 for all unpaid charges including court costs and attorney's fees incurred in the collection of these charges.

REPRESENTATION TO US: By signing this Agreement, I represent that I am the true owner of the above stated pet(s). I have read, understand, and completed this document and enter into this Boarding Agreement. I attest that all information I have provided about myself and my pet is complete, true, and accurate to the best of my knowledge.
This Agreement will remain in effect for all of my pet's visit's to HWK9.
