Requirement: Vendor Insurance Requirement for Juneteenth in Monmouth Night Market to ensure the safety and protection of all attendees, vendors, and community partners, all participating vendors are required to carry a minimum of $1 million in liability insurance coverage. Your policy must list the following as additionally insured:Additionally Insured Entity: Juneteenth in Monmouth at Main Street Park Location: Main Street Park, 120 Main St. E, Monmouth, OR 97361 Please ensure your coverage is current and that a copy of your Certificate of Insurance (COI) is emailed to Culturalharvestcollective@gmail.com prior to the event. This is a mandatory requirement for participation and helps us keep the event safe and compliant for everyone involved.

Food Vendor Requirements: Licensing & Compliance All food vendors must: Have proper licenses and food handler’s cards. Meet all local/state regulations. Temporary Restaurant LicenseAll food vendors must apply for a temporary restaurant license (if required through OHA requirements as a food vendor) for each event in POLK county BEFORE June 1st.

WAIVERIN CONSIDERATION OF THE GRANTING OF PERMISSION TO PARTICIPATE IN THE MONMOUTH JUNETEENTH NIGHT MARKET, THE VENDOR AGREES TO ASSUME ALL RISKS ASSOCIATED WITH PARTICIPATION, INCLUDING BUT NOT LIMITED TO, THE USE OF FACILITIES, EQUIPMENT, AND ALL HAZARDS RELATED TO THE OPERATION OF THEIR BOOTH. THE VENDOR HEREBY RELEASES AND AGREES TO INDEMNIFY THE CULTURAL HARVEST COLLECTIVE, THE CITY OF MONMOUTH, AND ALL AFFILIATED ORGANIZATIONS, INCLUDING THEIR DIRECTORS AND MANAGEMENT, FROM ANY AND ALL CLAIMS, DEMANDS, ACTIONS, OR CAUSES OF ACTION ARISING OUT OF THEIR PARTICIPATION IN THE COLLECTIVE. THE VENDOR IS ALSO RESPONSIBLE FOR OBTAINING AND MAINTAINING ALL REQUIRED LICENSES AND PERMITS, AS MANDATED BY THE STATE OF OREGON, APPLICABLE TO THE GOODS SOLD.I HAVE READ AND AGREE TO ABIDE BY THE REGISTRATION GUIDELINES, VENDOR AGREEMENT, AND ANY ADDITIONAL STANDARDS SET FORTH BY THE CULTURAL HARVEST COLLECTIVE, THE CITY OF MONMOUTH, AND ALL AFFILIATED ORGANIZATIONS, INCLUDING THEIR DIRECTORS AND MANAGEMENT.
