We hereby release and agree to save and hold harmless: Camp Nakanawa, Inc., RYLA Committee members, Rotary chaperones, Rotary leaders and members of participating Rotary clubs, as well as the sponsoring Rotary District 6780 and Rotary International from any and all liability which the student participant may or could claim or assert against any such person or Rotary entity, or any of their members, officers and directors by reason of any personal injury or death which could be suffered by said student participant, and for loss or damage to any personal property or any loss, monetary or otherwise, during the period of his/her stay and until their return to their home, expressly waiving any and all claims of liability at Camp Nakanawa, which the undersigned might or could assert. We also release and agree to have our student's images taken during the RYLA program and for those images to be used for any purpose whatsoever.

I understand that infraction of any rules, regulations and guidelines concerning improper behavior may cause my student or myself to be expelled immediately from the program.

Please complete this form online or mail it with your completed Registration Form, Rules and Guidelines and Adventureworks Release of Liability to the RYLA Coordinator of the Rotary Club who invited you. (See bottom of the Registration Form)
