MEDIA RELEASE AND AUTHORIZATION STATEMENT
I, the undersigned participant, hereby grant permission to Impact Activities, its employees, coaches, agents, representatives, and any authorized third-party media outlets to photograph, record, and use my name, image, likeness, and voice in any promotional, educational, or marketing materials. This includes, but is not limited to, use on websites, social media platforms, printed publications, and promotional videos related to Impact Activities programs and events.
I understand that all photographs, recordings, and content created shall remain the property of Impact Activities and may be edited or modified at its discretion. I waive any right to compensation, royalties, or approval for such use, now or in the future, and release all claims relating to the use of this media.
INFORMED CONSENT, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK
I, the undersigned participant, acknowledge and understand that participation in tennis programs, training sessions, and recreational activities offered by Impact Activities involves inherent risks. These risks may include, but are not limited to, physical exertion, falls, contact with equipment or other participants, weather conditions, and the actions or negligence of coaches, staff, or fellow participants.
By signing this agreement, I voluntarily choose to participate in these activities with full knowledge and acceptance of the associated risks. I confirm that I am physically and mentally capable of participating safely and that I have disclosed any relevant medical conditions that could affect my participation.
In consideration for being allowed to participate in these activities, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless Impact Activities, its owners, officers, employees, agents, representatives, affiliates, successors, and assigns ("Released Parties") from any and all liability, claims, demands, damages, causes of action, or expenses (including attorneys’ fees) arising out of or related to any injury, illness, death, or property damage that may occur before, during, or after participation—whether arising from ordinary negligence or otherwise.
I understand that this release does not waive liability for gross negligence, recklessness, or intentional misconduct as defined under California law.
INDEMNIFICATION AGREEMENT
I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, liabilities, or expenses resulting from my actions, participation, or failure to follow safety instructions while participating in any Impact Activities program or event. This includes claims made by third parties or other participants as a result of my conduct.
EMERGENCY MEDICAL AUTHORIZATION
In the event of a medical emergency, I authorize Impact Activities and its representatives to obtain emergency medical care on my behalf. I understand that reasonable efforts will be made to contact my designated emergency contact. I accept financial responsibility for any medical treatment provided.
PAYMENT AUTHORIZATION
I authorize Impact Activities to charge the credit card associated with my Impact Activities account to secure my camp registration. I acknowledge that registration is not confirmed until payment is received. I understand that all cancellations or withdrawals are subject to the stated cancellation and refund policy.
ACKNOWLEDGMENT AND GOVERNING LAW
I have read and understand this agreement and enter into it voluntarily. I acknowledge that by signing this form, I am giving up legal rights and remedies that may be available to me.
This agreement shall be governed by and interpreted in accordance with the laws of the State of California. Any legal action or proceeding arising from this agreement shall be brought in a court of competent jurisdiction in San Diego County, California.
By signing below, I affirm that I am at least 18 years old, legally competent to enter into this agreement, and fully understand its contents.
