By registering for and/or attending the Outdoor Dance & Movement Celebration hosted by Alacarte Professional Services, DBA Healing-Informed Art Center, in partnership with Take Action LA, I acknowledge and understand that participation in physical activities, including dancing and outdoor movement activities, involves inherent risks, including but not limited to slips, falls, physical injury, or other unforeseen incidents.
I voluntarily assume full responsibility for any risk of injury, loss, or damage that may occur as a result of my participation or the participation of any minor child under my supervision.
I hereby release, waive, discharge, and hold harmless Alacarte Professional Services, DBA Healing-Informed Art Center, Take Action LA, event organizers, volunteers, staff, contractors, partners, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained while participating in or attending this event.
I understand that this event takes place outdoors and that participants are responsible for wearing appropriate attire and footwear suitable for physical activity.
I further acknowledge that all minors must be accompanied and supervised by a parent or legal guardian at all times, and that the parent/guardian assumes full responsibility for the minor’s participation.
