I give consent to the processing of my personal data by the Kielce Cultural Center for the organizational purposes connected with the 25st International Competition for Fashion Designers and Enthusiasts OFF FASHION, pursuant to Art. 6 section 1 of the Regulation of the European Parliament and the Council /EU/ no. 2016 679 dated 27.04.2016 on protection of individuals with regard to the processing of personal data and on the free movement of such data and I hereby declare to have read the information clause provided on the website of the Kielce Cultural Center.

Wyrażam zgodę na nie odpłatne rozpowszechnianie mojego wizerunku , na podstawie art.81 Ustawy z dnia 4 lutego 1994 roku o prawach autorskich i pokrewnych /Dz.U z 1994 roku w formie fotografii , materiałów informacyjnych gromadzonych na stronach internetowych Kieleckiego Centrum Kultury , albumów lub wydawnictw , oraz rejestracji audio-video na potrzeby organizacji konkursu.

I give consent to the free distribution of my image, pursuant to Art. 81 of the Act dated 4 February 1994 on copyright and derivative rights /Polish Journal of Laws dated 1994/ in form of pictures, information materials collected on web pages of the Kielce Cultural Center, albums or publishing, as well as in form of audio and video recordings for the organizational purposes of the competition.
Jeśli Projektant/ka jest osobą poniżej 18 roku życia, prosimy o dołączenie zgody Rodzica/Opiekuna na przyjazd i prezentację jego/jej projektu. Organizator podejmuje ostateczną decyzję w kwestiach spornych wynikających z interpretacji Regulaminu.

If a Designer is under 18 years of age, please attach a Parent/Guardian's consent for arrival and presentation of his/her design. The Organiser shall make a final decision in any contentious issues resulting from the interpretation of the Regulations.
