New Prairie Soccer Registration

This is to register your child for the New Prairie Soccer Club Spring Outdoor 2026 soccer season.
You will need to fill out one registration per child. At the end of the registration form you will be taken to our payment portal. Once you have completed the registration and payment you can start a new registration form for the next child.

Please be aware that occasionally the form has difficulty being submitted when it is completed using a smart phone. If you are having difficulty, please try it on an Ipad or desktop computer and that should allow you to submit the form.

UNIFORM INFORMATION

For a uniform, each child will receive a reversible uniform top (navy blue/white), a pair of navy blue shorts, and a pair of navy blue game socks and white socks. The K-5th grade divisions use a different uniform than the middle school division, so please make sure that you purchase a new uniform if your child just moved to the middle school division and does not have the correct uniform. If your child has a uniform already, please make sure to select, "NO UNIFORM REQUIRED."
Shirt Size - Child will receive the same size shorts as shirt size selected. Please select "NO UNIFORM REQUIRED" if your child already has a uniform*

By submitting the form, I, as the parent/guardian of the registrant, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may given under whatever conditions are necessary to preserve the life, limb, or well-being of registrant.

By submitting the form, I, the parent/guardian of the registrant, a minor, agree that I and the registrant will abide by the rules of the New Prairie Soccer Club, its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with soccer and in consideration for the NPSC, accepting the registrant for its soccer programs and activities (the "Programs"). I hereby release, discharge and/or otherwise indemnify the NPSC, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the Programs, against any claimby or on behalf of the registrant as a result of the registrant's participation in the Programs and/or being transported to or from the same, which transportation I hereby authorize.
