This Warranty Registration Form, completed with all the required information, must be submitted along with a bill and a photo of the code on the back side of the roofing panel to Worthouse no later than 90 (ninety) days from the product purchase date to register the warranty.
By submitting this form you are agreeing that Worthouse. Metal Roofing Manufacturing has permission to reach out to the contacts on this forms regarding the warranty registration, the product purchased, and their experience with Worthouse Meal Roofing Manufacturing.
Worthouse provides a Limited Warranty for products sold by Worthouse, in accordance with the General Warranty Terms available on the Worthouse website www.worthouse.com. Please note that the installation of the product is not covered by this warranty.
