MEMBERSHIP TERMS AND CONDITIONS
By signing this agreement, the Member agrees to the following terms and conditions for membership with DMS Farm:
1. Membership Plan Selection
The Member selects the following membership plan:

2. Payment Terms
Membership fees are paid on a monthly basis.
The first payment is due upon signing this agreement.
Payment is due on the 1st day of each month thereafter.
Each monthly period runs from the 1st to the end of that current month.

3. Membership Sessions
The Member will receive ________ sessions per month as part of their selected plan.
If the Member starts membership mid-month, sessions may be used until the end of that current month in which they sign up.
Sessions do not roll over to the next month.
Any unused sessions must be used prior to the end of the month or they will be forfeited.
4. Membership Duration and Cancellation
This membership is valid for one (1) year from the date of signing.
To discontinue membership, the Member must provide one (1) month written notice prior to the end of the annual membership period.
Membership will automatically renew for subsequent one-year periods unless proper notice is given.
5. Payment Method
The Member authorizes DMS Farm to charge the following payment method for monthly membership fees:

6. Member Responsibilities
The Member agrees to:
Provide accurate and complete information on this form
Notify DMS Farm of any changes to contact or payment information

Use membership sessions in accordance with farm policies and scheduling procedures
Comply with all farm rules, safety requirements, and guidelines
7. Farm Responsibilities
DMS Farm agrees to:
Provide the agreed-upon number of monthly sessions
Maintain facilities and services according to established standards
Process membership fees on the agreed schedule
Provide at least 30 days notice of any changes to membership terms or pricing
8. Liability Waiver
The Member acknowledges that farm activities may involve certain risks and agrees to participate at their own risk. The Member releases DMS Farm, its owners, employees, and agents from any liability for injuries or damages that may occur during membership activities, except in cases of gross negligence or willful misconduct.
9. Agreement Modification
DMS Farm reserves the right to modify membership terms with 30 days written notice to the Member. The Member may cancel membership within 14 days of receiving notice of material changes without penalty.
10. Governing Law
This agreement shall be governed by the laws of the State of
