  • Football is an athletic activity which involves body contact between participants. While every precaution is taken to avoid injury, the risk of injury is always present. Additionally, every effort is made to ensure all coaches within our league are well trained and are permitted to work with children. The above-named player has my permission to play football and to participate in all practice sessions and games for the 2026 season. By signing this form, I waive any and all causes of action which may arise in connection with or incidental to the player's participation in this sport. I further waive any and all claims against the CFA and its officers for any unforeseeable event or for injury that may occur and recognize that if any of the above listed information is false, the player will immediately be disqualified from participating in the CFA Football league.

  • Authorization for Medical Care

  • I authorize any league and/or team official to act for me in my absence to use his/her best judgment in the event of a medical emergency requiring medical attention. I hereby waive my right to bring any claim against such individual in the exercise of such judgment. I recognize that insurance coverage for injuries received during the 2026 season is the responsibility of the parent or guardian's insurance policy.

  • CFA Player Registration Form Instructions

  • Please print clearly on this form. A copy must be placed in the CFA roster book (with the player's birth certificate, physical examination, and other pertinent information). A computer-generated list of players using these forms must be developed using either the CFA Website or Microsoft Excel Software. The file must be placed in the association player roster book, and also forwarded to the CFA Commissioner for posting on the CFA Web site.

    • Registration cost is NON-REFUNDABLE
    • Payments will be accepted online during Registration (Card Transactions)
    • Upload or bring a copy of your child's MOST RECENT PHYSICAL to 1st practice (July 27th, 2026)- Physical must be completed after July 27, 2025 to be eligible for use

  • Football Registration Cost:

    • Early Bird (March 1, 2026-April 30, 2026):
      • Football- $105.00
    • Regular Registration (May 1, 2026-June 30, 2026):
      • Football- $130.00
    • Includes:
      • Uniform rental
      • Equipment rental
      • Practice Jersey Rental
    • Players will be required to provide their own Cleats, Mouthpiece and Black Practice pants
  • Cheer Registration Costs:

    • Early Bird (March 1, 2026-April 30, 2026):
      • $160.00
    • Regular Registration (May 1, 2026-June 30, 2026):
      • $185.00

  • Returning Midget Cheerleaders Registration fee is waived- Price for returning Midget Cheerleader is $55.00 to purchase, Sneakers, Bloomers and Bow.

  • Tri-Community Youth Sports (TCYS) Bylaw Agreement

  • "Zero Tolerance Spectator Rules of Conduct"

  • Youth Football is for the development and participation of young children. Football games are contests that allow our children to learn to honor the game of Football. On the field officials penalize players and coaches when rules are violated, and behavior is unsportsmanlike. Off the field, there are no officials to penalize spectators whose behavior becomes unacceptable due to stress and emotions.
  • In order to protect the reputation and the participation of our children, hereby adopts the following to be enforced by all CFA Associations.
  • ZERO TOLERANCE SPECTATOR RULES OF CONDUCT

    • No Profanity is allowed.
    • No intimidating language or actions shall be directed toward players, coaches, spectators, guests, or anyone else.
    • No heckling of game officials, coaches, or players
    • Absolutely no spectators shall be allowed inside the fence during practices or on game days.
    • Absolutely no smoking or alcoholic beverages are permitted during practice or games.
  • The unacceptable behavior of a single spectator could endanger the continued participation of our entire group of children in this league. Consequently, any person who violates any one or all of the "Zero Tolerance Spectator Rules of Conduct" will cause themselves and possibly their participating child/children to be immediately expelled from the league.
  • I agree with the terms and conditions of the Tri-Community Youth Sports Bylaws.
  • I understand that failure to comply with the bylaws mentioned above will result in expulsion from the Tri-Community Football League for the season.

  • Photo and Social Media Release

  • I grant Tri-Community Youth Sports Association my permission to use the photographs and or videos taken this 2026 football season for any legal purposes or promotional use, including but not limited to: use on our social media platforms, publicity, copyright purposes, illustration, advertisement, and web content. Furthermore, I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.

  • Fundraising Efforts:

    • In order to keep our registration costs lower this year, we will need full participation in at least 2-3 fundraisers.
      • Failure to participate in fundraisers or pay $50 buyout per athlete may impact on the athletes play time for 1 game.
  • I understand that as a participant in the Tri-Community Youth Sports Association for the 2026 season, I am required to participate in at least 2 fundraisers.

  • PARENT & ATHLETE CONCUSSION INFORMATION SHEET

  • WHAT IS A CONCUSSION?

  • A concussion is a type of traumatic brain injury that changes the way the brain normally works. A concussion is caused by a bump, blow, or jolt to the head or body that causes the head and brain to move quickly back and forth. Even a "ding," "getting your bell rung," or what seems to be a mild bump or blow to the head can be serious.
  • WHAT ARE THE SIGNS AND SYMPTOMS OF CONCUSSION?

  • Signs and symptoms of concussion can show up right after the injury or may not appear or be noticed until days or weeks after the injury.
  • If an athlete reports one or more symptoms of concussion after a bump, blow, or jolt to the head or body, s/he should be kept out of play the day of the injury. The athlete should only return to play with permission from a health care professional experienced in evaluating for concussion.
  • DID YOU KNOW?

    • Most concussions occur without loss of consciousness.
    • Athletes who have, at any point in their lives, had a concussion have an increased risk for another concussion.
    • Young children and teens are more likely to get a concussion and take longer to recover than adults.
  • SYMPTOMS REPORTED BY ATHLETE:

    • Headache or "pressure" in head
    • Nausea or vomiting
    • Balance problems or dizziness
    • Double or blurry vision
    • Sensitivity to light
    • Sensitivity to noise
    • Feeling sluggish, hazy, foggy, or groggy
    • Concentration or memory problems
    • Confusion
    • Just not "feeling right" or is "feeling down"
  • SIGNS OBSERVED BY COACHING STAFF:

    • Appears dazed or stunned
    • Is confused about assignment or position
    • Forgets an instruction
    • Is unsure of game, score, or opponent
    • Moves clumsily
    • Answers questions slowly
    • Loses consciousness (even briefly)
    • Shows mood, behavior, or personality changes
    • Can't recall events prior to hit or fall
    • Can't recall events after hit or fall
  • "IT'S BETTER TO MISS ONE GAME THEN THE WHOLE SEASON"
  • CONCUSSION DANGER SIGNS

  • In rare cases, a dangerous blood clot may form on the brain in a person with a concussion and crowd the brain against the skull. An athlete should receive immediate medical attention if after a bump, blow, or jolt to the head or body s/he exhibits any of the following danger signs:
    • One pupil larger than the other
    • Is drowsy or cannot be awakened
    • A headache that gets worse
    • Weakness, numbness, or decreased coordination
    • Repeated vomiting or nausea
    • Slurred speech
    • Convulsions or seizures
    • Cannot recognize people or places
    • Becomes increasingly confused, restless, or agitated
    • Has unusual behavior
    • Loses consciousness (even a brief loss of consciousness should be taken seriously)
  • WHAT SHOULD YOU DO IF YOU THINK YOUR ATHLETE HAS A CONCUSSION?

  • 1. If you suspect that an athlete has a concussion, remove the athlete from play and seek medical attention. Do not try to judge the severity of the injury yourself. Keep the athlete out of play the day of the injury and until a health care professional, experienced in evaluating for concussion, says s/he is symptom-free and it's OK to return to play.
  • 2. Rest is key to helping an athlete recover from a concussion. Exercising or activities that involve a lot of concentration, such as studying, working on the computer, and playing video games, may cause concussion symptoms to reappear or get worse. After a concussion, returning to sports and school is a gradual process that should be carefully managed and monitored by a health care professional.
  • 3. Remember: Concussions affect people differently. While most athletes with a concussion recover quickly and fully, some will have symptoms that last for days, or even weeks. A more serious concussion can last for months or longer.
  • WHY SHOULD AN ATHLETE REPORT OF THEIR SYMPTOMS?

  • If an athlete has a concussion, his/her brain needs time to heal. While an athlete's brain is still healing, s/he is much more likely to have another concussion. Repeat concussions can increase the time it takes to recover. In rare cases, repeat concussions in young athletes can result in brain swelling or permanent damage to their brain. They can even be fatal.
