LIABILITY WAIVER- The undersigned being the lawful parent and/or guardian of the above child hereby consent to the participation by the child in all activities conducted by Washington Community Development Center and to the participation of the child in all events related to said activities. The child that participates in activities and adventures with Washington Community Development Center's activities and adventures and agrees to release, indemnify, defend, and forever discharge Washington Community Development Center and its subsidiaries, vendors, staff, employees, and agents of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action in participation in Washington Community Development Center. By signing this I give my child(ren) permission to play baseball in the Washington Community Development Center T-Ball League.
