TEAM "Kids Who Care" Registration Form

This registration form is for children age 12 and under participating in Hospice Georgian Triangle’s Hike & Bike on May 24, 2026. When you complete this form, your child will automatically be added to the Kids Who Care Team. Please provide all participant details, select a T-shirt size, and complete the waiver agreement. A Kids Who Care Fundraising Confirmation email will be sent to the parent or guardian. Participant packages can be picked up at the event registration desk on event day. All children must be accompanied by a registered adult participant at all times. Parents or guardians must complete a separate adult registration form.
