    Please read the instructions for bringing prescription medications to youth camp and acknowledge below.

  • If your camper will be bringing prescription medication to youth camp, please place all medications in one sealed Ziploc bag clearly labeled with the camper’s full name. Each medication must be in its original pharmacy packaging with the correct name, dosage, prescribing information, and date printed on the label. Upon arrival, the Ziploc bag should be turned in to your church youth leader, who will submit it to the camp nurse. All prescription medications will be securely stored and administered by our camp nurse according to the instructions provided. Medications will be returned to the camper at the conclusion of camp.

  • In the event of minor injuries or common discomforts (such as scrapes, headaches, or stomach aches), our camp nurse may administer basic over-the-counter medications as needed. These medications will only be given in appropriate dosages and under adult supervision to ensure the safety and well-being of each camper.   • By signing this document, I acknowledge that participation in activities at Calvary Youth Camp involves inherent risks, including but not limited to physical injury, illness, or property damage. These activities may include, but are not limited to, sports, outdoor adventures, swimming, crafts, and transportation to and from off-site locations.

    Assumption of Risk: I, the undersigned, voluntarily assume all risks associated with my/my child’s participation in camp activities. I understand that while Calvary Youth Camp takes reasonable precautions to ensure the safety of participants, it is impossible to eliminate all risks.

    Release and Waiver: In consideration of the opportunity for myself/my child to participate in Calvary Youth Camp, I hereby release, waive, and discharge Calvary Youth Camp, its officers, directors, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me/my child or to any property belonging to me/my child while participating in camp activities.

    Indemnification: I agree to indemnify and hold harmless Calvary Youth Camp, its officers, directors, employees, agents, and volunteers from any claims, demands, actions, or causes of action arising out of my/my child’s participation in camp activities, whether caused by negligence or otherwise.

    Medical Consent: In the event of an emergency, I authorize Calvary Youth Camp to secure necessary medical treatment for me/my child. I understand that I am responsible for any costs incurred for such treatment.

    Photograph/Media Release (Optional): I consent to Calvary Youth Camp using photographs, videos, or other media of me/my child taken during camp activities for promotional or educational purposes.
